Provincial Stanford Chronic Condition Self Management Project

The Calgary Health Region is leading an on-line self management pilot project whereby self management workshops will be offered on-line over a 6 week period to adults who currently suffer from a chronic condition. All workshops will be lead by two Stanford Trained Lay Leaders. To be an on-line moderator, you must be a Stanford trained lay leader and have access to high speed internet. On-Line moderator training will be offered early in the New Year with the first on-line workshop being offered in March. Each on-line moderator will be paid $400 CDN per workshop. Provincial Stanford Chronic Condition Self-Management Leader Training Program

Two more years of Provincial support!

Alberta Health and Wellness, in collaboration with Chinook Health are pleased to announce $300,000 in funding over two years to continue support of the Provincial Stanford Chronic Condition Self-Management Leader Training Program. Strengthening upon past successes, this opportunity will not only maintain those achievements, but maximize future provincial coordination and sustainability efforts. A made-in-Alberta approach will support and develop Self-Management Leaders and also increase awareness of the importance of self-management within chronic care service delivery in all 9 health regions. This initiative is one component of Alberta Health and Wellness’ support to help health regions develop their chronic disease management capacity using evidence based best practices.


Targeted efforts will build upon current strengths and assets, promote cross-sectoral cooperation and collaboration among health regions, public health agencies, community organizations, and family physician practices in Primary Care Networks. To increase capacity in Alberta, a third provincial candidate will become certified as a T-trainer (a trainer of Master-Trainers). Additionally, support will be provided for program and infra-structure development, and for Leaders and Master-Trainers. The recently launched website (http://www.health-in-action.org/Chronic) will be maintained, updated and eventually re-developed to capitalize on technological opportunities to support a roster of provincial Leaders and Master-Trainers widely dispersed across Alberta.


Patient self management is a critical untapped resource in healthcare improvement. This provincial initiative will support and maximize on the strengths of self management by providing a supportive context for positive self-management through exemplary leadership and modeling of Leaders, Master-Trainers and T-Trainers.


Under the expert coordination of Chinook Health, this provincial program will ensure all nine health regions have equitable access to opportunities. The project coordinator will be re-connecting, in the near future, with regional key stakeholders to continue the momentum.
